Notary Supplies Order Form

Whether you have just obtained your notary commission or have renewed your commission, you will need basic notorial supplies such as a seal and stamp. You may order these supplies through the Dayton Bar Association and they will be delivered to your door.

To order notary supplies, please print and complete the Notary Supplies Order Form and mail it to the DBA with your payment or fax to us with your credit card payment at 937.222.1308.

You may also place your order at the DBA office when you pick up your new or renewal commission.
