Changing Your Name or Address After Obtaining Your Notary Commission

Name Change
To change the name on your commission, you must appear in person at the DBA office between the hours of 9:00am and 1:00pm, Monday through Friday. You must bring your old commission. A fee of $20 is due to the DBA office. MasterCard, Visa, cash in the exact amount and checks made payable to the Dayton Bar Association are accepted. Please keep in mind that the expiration date on your commission will change.

Alternatively, you can simply sign your married (current) name followed by "nee 'MAIDEN NAME'" when notarizing.

Relocation
If you have moved to Montgomery County from another Ohio county, you must record your commission with the Montgomery County Clerk of Courts (no renewal test is necessary). However, there is a $15 filing fee payable to the Montgomery County Clerk of Courts. The Clerk of Courts is located in Room 104, County Courts Building, 41 N Perry, Dayton, at the corner of Second & Perry in Dayton (937-225-4512).

You must also notify the Commission Clerk for the Governor giving your name, expiration date, and both your old and new addresses. Send this information to: Notary Commission Clerk, Ohio Secretary of State, PO Box 1658, Columbus, OH 43216-1658.
